Passport Information

The Lebanon Town Hall is a passport acceptance facility. The Administrative Assistant in the Selectman’s Office serves as a passport acceptance agent which you are required to see if you are a first-time applicant, your passport has been expired for more than five years, you were under 16 years old when your passport was issued, or your previous passport was lost, stolen, or damaged.

Routine processing times for passports tend to run around 6 to 8 weeks. If you need your passport sooner than that, you must pay to expedite the application. If you need a passport in less than 2-3 weeks, you must visit the Connecticut Passport Agency in Stamford.

Please note, we cannot accept passports less than 30 minutes before close of business. We also recommend calling ahead or making an appointment as we cannot always guarantee the availability of the acceptance agent. On-site photography is not available at the Town Hall.

Fees

The cost to apply for a Passport Book is $110.00 for adults (considered anyone 16 years or older) plus a $35.00 processing fee which is paid to the Town. Minor applicants cost $80.00 plus the $35.00 processing fee.

Applicants may also choose to purchase a Passport Card which costs $30.00 for adults and $15.00 for minors. Passport Cards allow you to enter the United States from land border crossings and sea ports-of-entry from Canada, Mexico, the Caribbean, and Bermuda. It cannot be used for international air travel.

Applications must be paid for by check or money order. The application fee and processing fee must be separate checks. Applicants applying in groups must submit separate checks for each passport application but are free to pay for their processing fees with a single check.

The fee to expedite a passport (get it in less than 6-8 weeks) is $60.00. Overnight return service costs an additional $16.48.

What to Bring (Applicants Age 16 or Older):

  • One check per applicant made out to the US Department of State for $30 for a Passport Card, $110.00 for a Passport Book, or $140.00 for both a Passport Book and Card. Checks should be made out to the US Department of State. Please write your Last Name, First Name, and Date of Birth (MM/DD/YYYY) on the memo line.
  • At least one additional check made out to the Town of Lebanon in the amount of $35.00 per applicant in your group or family
  • A DS-11 Passport Application which can be filled out online here or picked up in the First Selectman’s Office
  • A 2” x 2” square color photograph of yourself taken in the last six months. Passport photos can be taken and printed at many local pharmaceutical stores such as Walgreens and CVS. Do NOT staple your own photograph to the application
  • Primary evidence of U.S. Citizenship such as a birth certificate or undamaged U.S. passport (can be expired). Birth certificates MUST include a raised seal by the issuing authority and the full names of both parents
  • Photo identification such as an expired U.S. passport or a driver’s license
  • Applicants between the ages of 16 and 17 must demonstrate parental awareness by having at least one parent present to sign off during the application process or a signed, notarized statement of consent from at least one parent plus a photocopy of the parent’s ID

What to Bring (Applicants Under 16):

  • One check per applicant made out to the US Department of State for $15.00 for a Passport Card, $80.00 for a Passport Book, or $95.00 for both a Passport Book and Card. Please write the child’s Last Name, First Name, and Date of Birth (MM/DD/YYYY) on the memo line
  • At least one additional check made out to the Town of Lebanon in the amount of $35.00 per applicant in your group or family
  • A DS-11 Passport Application which can be filled out online here or picked up in the First Selectman’s Office
  • A 2” x 2” square color photograph of the child taken in the last six months. Passport photos can be taken and printed at many local pharmaceutical stores such as Walgreens and CVS. Do NOT staple the child’s photograph to the application yourself.
  • Primary evidence of U.S. Citizenship such as a birth certificate or undamaged U.S. passport (can be expired). Birth certificates MUST include a raised seal by the issuing authority and the full names of both parents.
  • Evidence of parental relationship such as the birth certificate, adoption decree, or divorce/custody decree
  • Photo identification for both parents/guardians
  • Parental consent from both parents by appearing in person at the time of the application. If both parents are not available to appear, please consult Section 7 of the State Department’s guide to applicants under 16, available here.
